CORRECTIONS! Indoor Drumline – LCPS Required Forms

 

Hello Indoor Drumline Parents and Students!

       Please except my apologies! This email contains two corrections:

  1. Submission email error has been corrected.
  2. Correct LCPS School Day Extended Day Field Trip Permission Form for Winter Season has been linked.

The following documents are required by LCPS for all Indoor Drumline  participants and should be completed by a parent and submitted as soon as possible. For convenience, the forms may be completed electronically and submitted to studentservices@brhsbba.org via email OR forms may be printed out, completed, and turned in to Mr. Dempsey.

#1 – Acknowlegment of Risk Form

Please complete the form below for all Indoor Drumline students.

#2 – School Day-Extended Day Field Trip Permission Form

Please complete the form below for all Indoor Drumline students.

#3 – Medication Authorization Forms

Please complete a Medication Authorization form if dispensing of medication is required and your student didn’t march in the fall with the Marching Spartans or if there is a change in medications since the fall.

Medication Authorization forms are required for all Prescription and Over the Counter medications. Any medication must be provided for the student by the parent and must be delivered directly to the Student Services/Mr. Dempsey by the parent. We cannot accept medication directly from a student.

The following document is required by LCPS if your student will need to take Prescription medication throughout the season during rehearsal or while traveling to and performing at competitions. Your doctor must complete and sign this form.

For students who take Over the Counter (OTC) Medication, please read our Cover Letter for Medication Administration of Common OTC Medications to see if this form may apply for your student:

  1. If your student has a chronic condition that requires regular dosing of one of the following medications (Diphenhydramine aka Benadryl, Ibuprofen aka Advil or Motrin, Triple Antibiotic Cream/Ointment aka Neosporin, Calcium Carbonate aka Rolaids or Tums, Loperamine aka Imodium, Cough/Throat Drops (aka Halls, Ludens, or Ricola), please use the following form:

  2. If your student has a chronic condition that requires them to take an Over the Counter Medication that is not on our Common OTC Medications form, please use the following form (only parent signature is required):

  3. For students who do not have a chronic condition who may need to take an Over the Counter Medication, we ask parents to please ensure that students take their medication at home or a parent may come and administer the medication during rehearsal or at an event themselves.

#4 – Student Accident Bulletin

Finally, LCPS offers Student Accident insurance for those who wish to purchase a policy. Please read through the following document and, if determined that this is a good fit for your family’s needs, submit to Sheli Fierstine and Katrina Towle.
